INVESTIGATION OF EUSTACHIAN TUBE DYSFUNCTION IN PATIENTS WITH CHRONIC RHINOSINUSITIS USING THE ETDQ-7 QUESTIONNAIRE

Abstract
Introduction: Eustachian tube (ET) is a common condition in patients with chronic rhinosinusitis (CRS). The Eustachian Tube Dysfunction Questionnaire-7 (ETDQ-7) is a useful tool to help doctors in diagnosing and evaluating Eustachian tube dysfunction (ETD) but has not been studied in Vietnam. Objective: The study aims to investigate ETD in patients with CRS using the ETDQ-7 questionnaire and evaluate the association between the ETDQ-7 score and the severity of CRS and the consensus between the results of measuring the function of ET and tympanogram. Materials and methods: The study cross-sectionally described 96 cases of diagnosis of CRS by EPOS2020, assessed the status of ETD by the ETDQ-7 questionnaire and assessed the function of the eardrum by measuring the volume of the eardrum and measuring the function of the eardrum from 01/2024 to 10/2024. Results: Average age 31.03 ± 9.64 years. The rate of male patients was 59.38%, the rate of smoking was 29.17%. The most common ear symptoms are full ears (71.88%) and tinnitus (52.08%). Common nasal symptoms are runny nose (91.67%), nasal congestion (68.75%). The average SNOT-22 score was 25.20 ± 8.99 points. The average Lund-Kennedy score is 6.20 ± 2.14 points. The average Lund-Mackay score is 8.20 ± 5.47 points. The average ETDQ-7 score was 14.59 ± 3.18 points. The number of cases of CRS with ETD accounted for a high rate of up to 46.88% based on the assessment through the ETDQ-7 scale. 30.21% of patients had abnormal tympanogram. 52.08% of patients had ETD based on measurement of ETD. The ETDQ-7 score agrees favorably with the measurement of ET function (Kappa=0.65) with a sensitivity of 78%, specificity of 86.69%, area under the curve of 0.83 (p<0.001) and has a linear correlation with the SNOT-22 score with mean strength (r=0.46, p<0.001; r2=0.2, β=1.31) and has no correlation with the Lund-Mackay and Lund-Kennedy scores. Conclusions: The ETDQ-7 questionnaire is a simple, valuable, and easy-to-use tool. Therefore, this tool will be very useful in assessing the status of CRS in patients with ETD and thereby helping to improve the quality of diagnosis and treatment in patients with CRS
