The result of treatment for post temporal bone fracture facial paralysis

Abstract

Objectives: This paper aim to evaluate the outcome of post temporal bone fracture facial paralysis treatment.


Material and methods: a total of 26 patients who had diagnosed post temporal bone fracture facial paralysis and treated at Hue Central hospital from 6/2020 to 10/2023. A prospective cohort study was conducted.


Results: Of the 26 cases, 65,4% were male. The main reason for trauma was traffic accidents. The mean age was 35,3. Longitudinal fractures were seen in 13 cases (50%), tranverse fracutures in 9 cases (34,6%), and mixed fractures in 4 cases (15,4%), 69,2% of the fracture impacted optic capsule. The result of topognostic Test was normal in most cases.  Facial paralysis was of immediate onset in 15/26 cases, most of the patients had severe dysfunction or total paralysis of facial nerve (House-Brackman grade V-VI). Conservation treatment was seen in 9/26 cases, 17 cases were treated with facial nerve depression. After 3 months of treatment, 76,9% patients recovered well (House-Brackman grade I-II).


Conclusion: Facial nerve decompresion is an effective method in patient with temporal bone fracture facial paralysis.
