Diagnosis and treatment of acquired middle ear cholesteatoma using the EAONO/JOS system

Abstract

Objctives: The aim of this study involves applying the EAONO/JOS system in diagnosis and treatment of acquired middle ear cholesteatoma. Methods: This was a cross-sectional study performed at Ear Nose Throat Hospital of Ho Chi Minh City between December 2020 and December 2022. Results: The discrepancies in staging of acquired middle ear cholesteatoma were found in 7/71 cases (9.86%) between the preoperative and intraoperative evaluation, with the Kappa score was 0.766, indicating the high validity of the EAONO/JOS staging system. The surgical technique was significantly associated with the disease stage (Fisher’s exact test, p = 0.006). However, the recurrence rate as well as the surgical time showed no such association (Fisher’s exact test, p > 0.05). Moreover, the recurrence rate showed no significant association with the difficult access sites of cholesteatoma (Fisher’s exact test, p > 0.05). Conclusion: The EAONO/JOS system is beneficial for the classification and staging of the acquired middle ear cholesteatoma preoperatively, as well as providing prognostic informations for difficulties during surgery. In addition, this system also helps the surgeons in planning the surgical technique (canal wall up or canal wall down) and the surgical procedure (microscope, endoscope, or combination of both) to obtain optimal outcomes.
