Abstract
CLINICAL, ENDOSCOPIC AND AUDIOLOGICAL CHARACTERISTICS BEFORE REVISION SURGERY FOR CANAL WALL-DOWN MASTOID CAVITIES
Objective: To describe the preoperative clinical, endoscopic, and audiological characteristics of patients with chronically discharging canal wall-down mastoid cavities who were indicated for revision surgery of the mastoid cavity. Methods: A descriptive case series was conducted in 50 patients at the National Otolaryngology Hospital. Preoperative data were obtained from medical records and standardized clinical examination, oto-endoscopy, and pure-tone audiometry. Results: Mean age was 46.01 ± 15.07 years and 70.0% were female. Otorrhea was present in 100.0%, tinnitus in 92.0%, and otalgia in 54.0%. Common cavity abnormalities included an irregular cavity (92.0%), an active/wet cavity (90.0%), a high facial ridge (80.0%), a narrow meatus and thin cavity skin (both 62.0%), and tympanic membrane perforation (54.0%). Of 50 patients, 38 had quantitative air-conduction PTA values; the mean air-conduction PTA in these patients was 54.5 ± 20.8dB. Profound hearing loss was identified in 16/50 patients (32.0%), defined as PTA >90 dB or an explicit medical-record diagnosis of profound hearing loss. Conclusion: Chronically discharging radical mastoidectomy cavities frequently coexist with unfavorable cavity morphology and substantial hearing impairment. Systematic preoperative assessment is important for revision planning and hearing rehabilitation.
