Clinical and Paraclinical Characteristics and Histopathological Correlation of Sinonasal Hemangiomas

Abstract

Methods: A retrospective descriptive case series was conducted on 62 patients diagnosed with sinonasal hemangioma at the National Hospital of Otolaryngology from January 2023 to March 2026.

Results: The mean age of the patients was 43,4± 19,0 years, with a 1:1 ratio between capillary and cavernous hemangiomas. The most common symptoms were epistaxis (61,3%) and nasal obstruction (51,3%). Grossly, the tumors appeared pink or dark red, with a smooth surface, and were highly prone to bleeding upon contact. No statistically significant difference in tumor morphology was observed between the two histopathological types (p > 0,05). However, a significant difference was found regarding the intraoperative tumor attachment sites (p<0,01). Capillary hemangiomas were predominantly attached within the nasal cavity, specifically at the inferior turbinate (32,3%) and the nasal septum (32,3%). In contrast, cavernous hemangiomas originated more frequently from the paranasal sinuses (61,3%), most notably the maxillary sinus.

Conclusions: Sinonasal hemangiomas are commonly encountered in middle-aged individuals, characteristically presenting with unilateral epistaxis and nasal obstruction. The histopathological types correlate strongly with the site of tumor origin: capillary hemangiomas typically attach within the nasal cavity, whereas cavernous hemangiomas are more frequently found within the maxillary sinus.
