A SCOPING REVIEW OF THE THESIS: EFFECTIVENESS OF THE PATIENT-CENTERED TEAM CARE MODEL

Abstract
Objective: Overview of the effectiveness of the patient-centered team care model. Method: A scoping review. Results: Of the 335 relevant articles, 11 articles that met the selection criteria were included in the study. These studies were conducted in many different countries, all showing the effectiveness of the patient-centered team care model. Conclusion: The patient-centered team-based care model shows great potential in improving management and treatment outcomes for many conditions, especially chronic diseases.
