INTRATYMPANIC CORTICOID INJECTION FOR SUDDEN SENSORINEURAL HEARING LOSS: A SCOPING REVIEW

Abstract
Background: Sudden sensorineural hearing loss (SSNHL) requires emergency medical treatment. This study aimed to review the use of intratympanic corticoid injection in the treatment of SSNHL. Subjects and methods: A literature search was conducted using Pubmed database from January 2018 to December 2023, using the appropriate keywords.
Results: 119 articles were found, in which 20 articles met the inclusion and exclusion criteria were selected. Three types of corticoids used are dexamethasone (4 mg/ml, 5 mg/ml, 10 mg/ml), methylprednisolone (40 mg/ml) and triamcinolone acetonide (40 mg/ml). Treatments included initial therapy (alone, or combined with systemic corticosteroids) and salvage.
Except for the posterosuperior angle, other locations on the tympanic membrane can be used for injection. The commonly applied injection frequency was once a day, and once every 2-4 days; 77.1% had injection duration within 2 weeks. Initial intratympanic injection, alone or combined with systemic corticosteroids and salvage, all resulted in hearing improvement with similar rates. The most common discomforts were otalgia and dizziness/lightheadedness. Conclusion: Intratympanic corticoid injection is an effective treatment method, and is well applied in both initial and salvage regimens.
