LONG-SEGMENT TRACHEAL STENOSIS TREATED WITH FASCIOCUTANEOUS FLAP AND SHORT-TERM STENTING: CASE REPORT

Abstract

Background: Long-segment tracheal stenosis (LTS) that has failed end-to-end tracheal resection and anastomosis can lead to persistent airway obstruction. Although numerous surgical techniques have been described for LTS repair, the management of recurrent LTS in adults following prior failed airway reconstruction remains challenging. We present a novel staged reconstructive approach using a synthetic bone-supported fasciocutaneous composite graft in a patient with refractory LTS.

Methods: The patient underwent staged tracheal reconstruction with a composite graft consisting of artificial bone tissue and a vascularized fasciocutaneous flap. Due to insufficient structural support from the graft alone, a temporary stent was placed to maintain airway patency. The patient was able to breathe spontaneously through the reconstructed tracheal segment immediately after stent removal 

Results: Successful airway reconstruction was achieved, combining pedicled flap reconstruction with synthetic materials. The stent was successfully removed, and the patient's respiratory function was restored.

Conclusion: This technique offers a promising single-flap solution for complex tracheal and adjacent soft tissue reconstruction, eliminating the need for secondary flap procedures. The efficacy in this case suggests the potential applicability of this technique in similar challenging scenarios.
