A STUDY OF 140 CASES ENDOSCOPIC SINUS SURGERY FOR CHRONIC FUNGAL SINUSITIS AT UNIVERSITY OF MEDICAL CENTER, HO CHI MINH CITY IN 2019

Abstract

Introduction:Chronic fungal sinusitis is an inflammation of the sinus mucosa which caused by fungus and often occurs in immunocompromised patients[1]. The disease accounts for 10% of all chronic sinus diseases with a tendency to increase rapidly, recently. Sinus endoscopic surgery is indicated for cases of persistent fungal sinusitis due to the long-term etiology factors that make conservative treatments unaffectedly.

Objectives: to describe clinical factors, surgical methods; pathology and the results of cases of chronic sinuses inflammation, which caused by fungi at ENT department of UMC.

Methods: Design: prospective study in One hundred fourty consecutive surgical patients were indicated to underwent endoscopic sinuses surgery due to chronic fungal sinusitis which did not respond to conservative treatment, gender and age as well as key symptoms documented for the study. Samples from sinus such as fungus, polyps, mucosae and mucin were collected for research purpose. Histopathologic study as well as post operative follow up also documented for study. 

Result: 140 patients (44 male / 96 female), mean age 54.5 with common symptoms: headache or facial pain (77,4%), nasal discharge (75%), nasal obstruction (55%), foul smell (20,7 %), hyposmia (16,4%), common physical symptoms: mucosal edema (86%), thick with high viscosity mucus (71.4%), nasal polyps (12.8%). Postoperative symptom reduction: 100%. Sinuses endoscopic examination after 3 months of surgery: good (71.42), medium (9.28), bad (1.42). Complications: no complications

Conclusion: For patients with chronic fungal sinusitis that do not respond to conservative treatment, sinuses endoscopic surgery used to remove fungus, restore normal physiology of sinus mucosa is a safe and effective treatment.
